Phil,

What we do is to append a suffix to the rev and mark the boards, e.g. Rev C becomes C3 (third rev of the C artwork). Same goes for the BOM and engineering master (in our MRP program). 

Firmware is another story.  You know software, its never etched in stone. Any code that might need to be changed in the future must have a way for the customer to download the update.
