For a rigid board, IPC-2222 section 9 gives you this.

For level A:
maximum hole to minimum lead diameter, no greater than 0.7 mm over minimum lead diameter minimum hole to maximum lead diameter, no less than 0.25 mm over maximum lead diameter

For level B:
maximum hole to minimum lead diameter, no greater than 0.7 mm over minimum lead diameter minimum hole to maximum lead diameter, no less than 0.20 mm over maximum lead diameter

For level C:
maximum hole to minimum lead diameter, no greater than 0.6 mm over minimum lead diameter minimum hole to maximum lead diameter, no less than 0.15 mm over maximum lead diameter

For square lead you take the diagonal as the lead diameter.
