typical space rules for copper are OK, unless you are using voltages and frequensies where capacitive coupling could be an issue. Pretty rare on planes. I tend to add a bit more clearance between planes, just because I can.
As for holes between planes: annular rings, NFP removal.... all those reasons why Dewey said "don't" instead of "it depends". Safest to just avoid it.
