I have seen PTH external annular ring copper dissolution during rework and confirmed by cross section but never seen the whole barrel missing.   What type of LF solder are we talking about, wave/hand solder.   Is there any damage to the PWB laminate.   Are other nearby PTH effected which would have seen the same type of temperature excursion.   What was the rework dwell time?

I would vote for copper dissolution due to lead free HAL. The HASL
process can quickly remove copper in the PTH. Lead free HAL Rework and
your assembly process may advance the condition. A microsection of a
discrepant PTH would answer that question readily.
