It depends.

There.  Somebody had to say it.

If it's an easy to fab board, no X-outs, the fab house should be able to do that.

But if you don't allow x-outs on more difficult fabs, the board price increase will more than offset the cost of the paste.  It's very easy to mark over the fiducials so the pick and place skips that board.  We aim for no more than 20% on any panel, and note it on the fab drawing as the allowable number of x-out boards.  With good vendors, that gives us a very low percentage of x-outs per lot, but still allows them to keep prices in control by not scrapping an entire panel for one missed board.
