Well I really feel fiducial's should be in a P&P file, they can be 
output or entered into that file in a format that the program
can be setup to recognize them as fiducial's and their coordinates
separate from actual parts.
I would want to have them in there even if for nothing else but to be a verification of the coordinates
of the fiducials. By the way an assembly dwg should specify & dimensionally locate where the 0,0 datum 
for provided assembly data is. Some also dimension fiducial locations on the assy dwg, which in that
case if programming does not want them in the file at least they are defined.

I was wondering about this, and I think what might be happening is that
an assembly house is panelizing my design, and adding fiducials to the
break-away strip.  Then they have all locations marked on their new PNP
file.  At least that's a guess, although perhaps, since they have the
gerber file too, they can just go in and locate the fiducial easily
enough, and enter that data into their PNP too.

I've never reset the (0,0) location; I usually wind up with lower left
corner at (1000,1000), although some boards I do it at center--"it
depends" upon where it goes, sometimes the mechanical drawings I get are
referenced to center, other times to a corner; and I usually just keep
that same reference.

I have never added Fiducials to my Pick and Place data.
My preference is to set 0,0 at a Fiducial and have the PnP written from
there.
Currently the tool I use does not make this easy so I add a dimension to
my Assembly drawing to show the offset from 0,0 to first Fiducial.
Fiducials are not parts so they don't belong in a PnP file.
Just another opinion,

Nah, using Altium.  I have them set as "Standard (No BOM)" and it
appears they don't come out on Pick and Place when I do that.  Did not
realize until I saw it today, by accident; most of my boards have
fiducials "just in case" it goes to pick and place.  This particular
board will be, so I'm glad I caught it now--but now I need to check on
that on a bunch of prior designs...

They should definitely be in the PNP file.

If you're using PADS, I recommend adding them as non-ECO parts and
giving
them a prefix like 'FID' so they can be spotted easily.
