Mike, thank you for answering this inquiry on behalf of IPC.  I must correct one statement, however, and that is that just about all of the largest North American PCB manufacturers do participate in the IPC statistical program.  

Global data is available from IPC.  Rather than duplicate the efforts of our sister associations in other countries, IPC partners with them and we share our aggregate data once a year to publish the World Electronic Circuits Council (WECC) World PCB Production Report.  IPC also publishes its own World PCB Production Report, which provides consensus estimates of the value of PCB production by country and by product category.  It is the most detailed report of global PCB production available.  

Both of these reports are available free to IPC members on IPC's members-only web site.  Just go to www.ipc.org/membersonly and log in with your email address and password.  Then click on IPC Market Research.

The book-to-bill ratio is calculated by dividing the value of orders
booked over the past three months by the value of sales billed during
the same period from companies in IPC's survey sample.

For the IPC survey, only N. American board fabricators are included (and
yes, I believe they have to be IPC members). IPC does not release the
names of the companies that participate; however, it is my understanding
that more than a few of the largest PWB fabs in N. America do not
participate. Dedicated board distributors/brokers are not allowed to
participate, yet since so many US/Canadian fabs now broker some amount
of boards, IPC asks them to report the percentage that are not built in
N. America.

Some other countries track their national PWB shipments -- Japan does
through the JPCA, and Germany through VDL, for example. There is no
single entity that aggregates all the various national data into a
single monthly figure, however.

how is the ratio arrived at? only from NA board makers who are members
of IPC?

how about the rest of the world, many of which are not members of IPC?
