On the hybrids I've been hearing on the news by the people that had the accelerator problem was that you can't put the car in neutral nor shut it off when it is going over 50 MPH.
It sounds like they need a kill switch that doesn't disable the brake system.
Check out one of the Toyota circuit boards, I believe it was related to the accelerator or the y wouldn't have taken the module apart.

most cars built today have drive by wire with hall effect sensors.  No one else is reporting problems on Toyota's scale.  Then again, everyone else uses  "brake wins " software.

You already have a kill switch installed on your Toyota, from the factory.  It's located on the steering column, on the right side, below the steering wheel.  
It's multiposition, Accessory, Engine On, Engine Off, Starter  ;-)

I'm having trouble finding numbers, but what percentage of these incidents have occured in the US vs. the ROW?

As for manufacturers assuming responsibility, my Dodge truck came with an engine that Dodge knows is designed in such a way that the oil absorbs moisture, creating sludge that eventually causes the engine to sieze.  New engine, $6K.  BTW, their comprehensive warranty denies claims from owner abuse, specifically listing "oil sludge" in the abuse symptoms.  (Thought I was a victim this week, turned out to be bad gas.)
