I've seen all kinds of crystal/oscillator failures.  Usually they have vapor deposited metallization, and often they are laser trimmed.  I've seen too much laser scribing.  I've seen failure of the conductive epoxy gluing on the crystals.  I've seen crystal fracture due to shock--usually when I drop a small board with a crystal oscillator on it, it lands on the crystal package!  Some have windows where you can see the shattered glass.

   Can the electrode plating be too thin resulting in a failure.
  Can the plating migrate over time, vibration, over to the low spots on the surface of the electrode affecting the ESR?
