As mentioned on subject does anybody has experience in how to fill in
IPC 1752 11.1(Material Composition Declaration).here is the link of the

appreciate you if you can share me the instructions of that.Ours
production is a cable assembly that consist of tin copper,different
colour pvc plactic,connector,terminal ,heat shrink tubing etc,
my questions are:
1.How to define the option of the Form Type ?  (Option contains:
Request/Reply or Distribute)
2.How to define the option of the Declaration Class ?(Class 1/2/3/4)
3.Should I need to submit the IPC 1752 11.1 form for each of
metrials(copper,terminals...) to customer?
4.It will be very nice if you could send me a copy of IPC 1752 11.1 form
which has been completed well.
