We designed some 60 GHz circuits on  Corning 7740 glass substrates. These 
are 800um thick and have epsilon about 4.5. One use to get on some 
nanometers Ti/Cu binder and then copper. Very fast signal substrates, low 
losses, little spurioses, little noise etc. May be something for you?

Coorstek looks promising. I have not been given packaging specs but expect 
the outline to be >>25mm square the since we are dealing with frequencies 
 >>10GHz.

Jeff,

Just one of multiples out there, but Coors is still into thin and thick
film ceramics.  I pasted their thin film ceramics link below because I
equate HF with thin film patterning and 99% alumina.  Whichever you
choose, they have it.

Laserage does laser processing of thick and thin film substrates if you
need holes cut, scribe lines for snapstrates, etc.  Don't recall if they
do dicing.  We do that inside so don't need to ask for that.  There are
multiple other laser houses out there too.
