I've seen PTH cracks in the past & I'd bow to the superior knowledge on the 
root cause already expressed on the matter. My own experiences of this 
problem is that its a time consuming & frustrating problem to diagnose & 
guarantee a fix, so I apply a series of design rules when laying out a PCB that 
tries to build in some fail safes (may not suit everyone but these work for me) 
in case the PCB fab is having an off day so my two cent's - 
 
 - Hole Aspect ration ¡Ü6:1
 - Copper wall thickness 30um Minimum
 - Inner layer redundant pad's must be retained
 - Annular ring of the inner layer pads to be 0,30mm (Drill + 0,60mm)
