See IPC/7711/7721A for details on how to attach wires to SMT PADS.  With 
respect to supporting wires to the PADS, if your equipment is subject to any 
shock or vibration requirement, I recommend you secure the end of each wire 
that connects to the PAD using some type of adhesive.  We use Dow Corning 
RTV 3145 which is a silicone based adhesive, and apply it within 1/2 inch of 
the wire end.
