I was referring to SMT, but thanks for the input. Looking @ my process again is my first step in all this. If I am getting good prints and the SMT ops are pushing boards through then at least I know that is covered. However when it comes to dealing with the people in the cells, well that is a whole different monster.

Short answer: no.
The problem is there is no one cause. If you do a fishbone diagram (man,
machine, materials and method) you will end up with a pretty big fish
for insufficients. And I didn't even see the original e-mail, so I am
not sure if you are talking about PTH or SMT!
So, obviously, your main thing is to have your process in "control". I
use quotation marks because it depends on what your level of tolerance
for errors is.
The second and less productive possibility is inspection. I would avoid
this as much as possible.
I know my reply deals more with philosophy than specifics, but I thought
it was worth saying.

to address the inspector issue.
Normally, inspectors do not cause rework. They have criteria from
J-STD-001
and ANSI/IPC-A-610. Based on that criteria they will determine if an
assembly
passes or fails the criteria. Unless that Inspector is quite
experienced, a
Quality Technician or Engineer, or a Process Engineer determines what
action
must be taken. Often "Insufficient Solder' is a process indicator. The
process
needs improved, but the board does not need to be reworked.
Incidentally, I don't think that the term "Insufficient Solder" is in
either
standard. I think that it is just a general term that we often use
without
much definition.
