We apply bar-code code labels as a value-add (we're a board shop) --
keep in mind that these labels can go over RF plane areas or dense runs
of tracking, with a little thought no real-estate need be wasted in most
instances...
Scratching has worked well for decades and continues to do so -- but for
a couple cents a board, it's not hard to relieve that itch...

Yes, scratch. Hey, it worked in the '70s and '80s. If it isn't broken,
why fix it? What you suggest is typically what we do. I was wondering
if anybody leaves, for example, a 0.38 x 0.75 inch empty space
specifically for a sticky label that might include some of the
information.

Do you mean that literally or writing something in ink? On a
multi-layer
board you are risking scratching through a layer of fiberglass to
copper.
The way I've done it for years is to put the assembly number on the
primary
side along with a blank block for the assembly revision and another
block
for the board serial number. And usually the company logo in silk.
The secondary side has the bare board part number and bare board
revision
level in copper. I think the reason for that is that the silkscreen has
a
chance of getting erased, but the bare board number in copper is almost
indelible.
I've seen every combo possible in 20+ years, including adding a blank
block
in the silk to add computer MAC addresses, ESD and lead free symbols,
both
the assembly and bare board in copper, both in silk etc.
If you like, I can send a jpg of the method so you can see what I'm
yammering about.

We are considering revising the way we mark our assembled boards.
Typically we want finished board assembly revision, assembly date code,
assembly part number, and if built by a CM an manufacturer's code.
Historically we have the basic part number in silkscreen on the
component side along with the word Rev with space for a revision letter.
We currently scratch the assembly dash number and revision into the
surface of the board.
Now with CMs doing some of the assembly they have other methods of
marking. In my former lives Engineering has been reluctant to give up
any board space for board identification. I'm interested in defining a
label area size and labeling method, e.g., polyester label, polyimide
label or epoxy ink stamp.
What sort of space do you leave for this kind of marking?
What sort of marking methods do you use?
