You do not need to apply for the RoHS exemptions, you just use them - either for the individual materials (like lead in ceramics) or if you are talking about computer servers, for the whole apparatus.

I am guessing that for "approval history papers" you are referring to the declarations from component suppliers for their components.  If so, you ask them to give them to you.
