I usually submit the one-up design, then ask the pwb vendor to optimize
the panel layout to get the maximum number of panels out of one of their
standard flat sizes.
Then, I may change the panel layout to optimize the panel size,
position, tooling hole location, fiducials, etc,  for the production
equipment, and I may also find better ways of scoring/routing to
maintain the required rigidity and minimize the depaneling time
required. But when I do this I try to stay within the panel dimensions
proposed by the board vendor. Then I run these past the board vendor
again. 
I also make sure no components will be overhanging a route or a score
line, etc. It requires a lot of communication and interfacing with the
board vendor, but it always pays off. DFM with FMEA in mind.
The good PWB vendors usually are very accommodating in spending a little
time planning the panel layout. They have a vested interest in making
sure their customer's board designs are as competitive as possible.

A quick question.  When designing small PWBs, do you design the paneled
array or do you supply a one-up design and ask your PWB supplier to
design the panel for you?
