Jim,
the main role of the panel is to allow for cheap assembly.
Never have the PCB fab do your panel by themselves! There are big chances that the assembly house will have troubles because of warpage, components too close to edges, excessive cutouts in the panel that disturb the sensors of the insertion machines, impossible depanelization, etc.
If you want to do the best job, you do the panel, by gathering input from both your assembly house and your PCB supplier.
It is important to have the panel done before even sending it out for quotation. Like this you will know that all the bidders will quote for the same thing. If not, one will quote a 1-up, the other one a 10-up, and so on, just to give you their best price. Imagine that you start with the fab that quotes a 10-up and then you change for the one that wants a 1-up: there will be trouble in the assembly room.
