The whole pricing structure is one of THE proprietary pieces of
information for a board contractor.  If they publicized their method of
pricing out a design, they could possibly be undercut a lot more easily
by a competitor.  
The same with SMT contract pricing.  Try to find out how much they
charge per component, etc?  No one will give you a straight answer.

The actual final cost formula is probably one composed of many different
aspects.  Use of the panel area, number of holes, number of hole sizes,
number and thickness of layers, type of material, pitch of features,
weight of the copper, final board finish, and whether they offer the
finish in their facility or if they have to contract any part of it out,
plus possibly some sort of minimum base price for really small basic
boards.

I second that, ballpark-wise (large volume - off shore). Last time I had
much real pricing data was a couple of years ago at least, and the rough
calc. I made at that time was slightly higher but board prices were
dropping.

On the other hand, I think the dollar was stronger then...

that's in the ballpark for volume offshore pricing I see regularly.
