This is actually another reason to let the PCB house decide on the drill
size.  They may opt to alter the drill size based on several factors -
size, geometry, aspect ratio, etc.

Doesn't this assume that plating in each of the holes is of uniform
thickness but at the same time isn't this rarely the case?  Does this
not
also mean that different holes sizes must be drilled to accommodate for
plating?
