Werner, the concerns with fractured barrel walls,
innerlayer separation, and etc. sound like a different
subject. Specifying the drill bit size or the finished hole
size is independant of those concerns. A good fab house
should have good hole wall quality and reliable plating
processes for whatever hole size they offer.

I do not have a problem with through-holes for component
leads and press-fit
pins be specified that way (lots of mechanical redundancy),
but I sure have
seen a lot of plated-through vias with fractured barrel
walls and
innerlayer-separation that were not properly spec'ed.
