Since this AM, we found one other dry hole close to the previous.  The
problem seems to be isolated to this particular connector on this
particular board, but there's no way to be sure without either x-ray, or
reflowing a bunch of other connections.

A while back, we soldered one board without spacers, and everything
looked fine.  It's possible that this particular spacer is wedged in
just right,leaving no way for air to escape up the hole during wave
soldering.

If that's wrong, and the problem is more widespread, it could well be
caused by heatsinking, either into a ground plane as you suggest, or
into the connector, like Werner says.  BTW David, these pins come in
with gold finish, and get pretinned as a matter of course.

I've been making some calls, and it seems like SBL X-Ray systems are
thinner on the ground than I thought.  Wish we hadn't junked ours years
ago.

any chance that this connector pin is going to a ground plane that's
sucking away heat from the plated through hole?

One of our boards has a number of 38999 connectors, with connections on
the top side hidden by a spacer.  We've had one SJ failure because of
inadequate solder fill up through the PTH.  We'd like to determine
whether this is an isolated problem, or whether the spacer might be
impeding solder flow.  Of course, we don't have any boards to sacrifice,
and we can't see anything with direct transmission x-ray because the
connector is in the way.

Anyone know of a lab somewhere nearby that offers scan beam laminography
service?
