Hi Carl,
The answers you already got [yes/yes/maybe] are likely correct.
However, it is more likely in you case, that the problems actually occured
when you started raising the input power. From you description, it did not sound
like that you sufficiently dried the assembly, and you started forming CAFs
[conductive anodic filaments] within the PWB going along the glass-fibers
between neighboring PTHs/PTVs. I bet you via density is the greatest [they are
closest together][ in the BGA area.
If this is the case, you cannot reverse the damage.
