I am assuming these are wave-solder through-hole joints? If so, my first
guess would be not enough topside pre-heat to allow the solder to flow
through, but upon further examination I realize that there is also a
large amount of voiding within the barrel, even when the solder does
flow through. Based on that, perhaps there is some type of contamination
inside the barrel. If you still have some unsoldered boards you may wish
to take a look at the cleanliness of the through-holes, and also inspect
for plating problems that could stop the solder wetting. Another
possibility is contaminated wave-solder flux, or flux with too high of
an alcohol content.
