Hello Mark, My name is Percy L. Mayes, I'm employed by Ametex-Dixson in
Grand Junction, Colo. 81503.  I've completed the course on
J-STD-001requirements on soldered electrical and electronic assemblies in
2003.
My certification will expire on June 06 of this year.    My registration
number is 3857, I was trained by Gregg Owens of MTTC in Monrovia
California.
  I am needing information on certification extension.  Could you direct
me or send me the forms that is necessary to apply for a 90 day extension?
