We apply the MSL rating for incoming PCBs, but I should qualify that the
PCBs are chip carriers or MCM substrates.

Our purchase requirements read something like "Package and ship per JEDEC
J-STD-033 Level 4 or equivalent".  What this does is require the board
supplier to dry bake and bag with desiccant and a humidity indicator card.
It prevents us from baking at the point of use.  History has shown this to
be beneficial from preventing delamination during soldering, improving wire
bonding, and improved electrical performance.

I'm a PCB fabricator, and I have a survey asking me several general
questions related to RoHS with one being moisture sensitivity level
(MSL)?  What exactly are they looking for? Does this pertain to bare
PCB's?
