how common is the flying probe test of the board prior to shipping?

They almost always never expect the board shop to pay for such
cost's...(laughing)

Seriously though, you do bring up a good question, I suppose it depends
upon...

1. The verification of the defect (i.e is it proven to be, or suspected to
be...)
2. Conformance to requirements (did it meet all requirements, but still
fail?...i.e. copper plating thickness met the minimum, but should have been
more copper)
3. Prior agreements, arrangements, etc between the supplier and customer
4. And probably a handful more that the room can toss in...
