John,
Normally that information is supplied on the PCB Fabrication drawing. I
don't think it is necessary to ask the fabricator where they get their
material from as long as it meets the requirements of the material that
is specified on the Fabrication drawing.

Sorry, I don't use a form. I select my materials and process by the type
of electronics I am designing.

Does any one have a form that would be given to a PCB Manufacturer to
have
them fill out to specify the make up of the PCB they are suppling. Seems
more often than not I need to drag the info out of them.

EX.  PCB material type, supplier and board material number
 Silkscreen type, material supplier and material number
 Soldermask type, material supplier and material number
 Solder type, material supplier and material number

 etcetera....
