First you must decide why you would allow LPI to be present anywhere except per the artwork.
If you are going to solder then you must reject all occurrences of mask in solderable holes/areas.
If you are not soldering then it becomes a process indicator that you will want to discuss with you PCB Fabricator.
Mask in vias is not a bad thing as vias are not considered solderable holes, also many designs today prefer mask filled or covered vias.
Slight occurrences of mask in mounting holes are not necessarily rejectable, but they can be an indicator that mask is being partially imaged and/or not being completely washed away and there fore may be also in the solderable holes.
Most important here is communication with your suppliers and customers to assure the end product meets all requirements outlined. If you do not care whether mask is in some holes but not others, then you can communicate this to your supplier to ease some of the restrictions we as designers and IPC place upon them.
Remember that communication solves most issues.
