Hi Russ,
In the past we used to had from time to time such phenomena. We found,that
the reason was not efficient rinsing living on the surface some residues
after drying.No enough to to spot them by ionic contamination tests, but
enough to create such high resistance short.We switched all final rinses
from  tap water to DI water as well improved maintanance of converised
equipment.
