Dave,
        Our Maintenance Guy poured a pad and ran
plumbing in about 8 hours total time.  I'm not sure
what we paid for the two bags of concrete, as they
were laying around from a previous project.  A little wood
framing, and the pad was complete.
        The plumbing was just straight through an outside
wall, and the freezer was in a room just inside said wall.  You
may need to consider more hours labor if your equipment is
in "the middle" of your building.
        We are using CO2 for a freezer, but I would assume
the costs would be comparable.
        We have a lease with a local company for about $500
per year, and about $90 for each re-fill.  I'm not sure what the
tank capacity is - its about the size of two 55 gallon containers
stacked.
        I'd say closer to a 1K investment, depending on you labor rates!
