The IPC 7711 and 7721 may address the repair and rework conditions you refer
to.  I think it's safe to say that services charges are determined by the
particular vendor who would provide the service.  They probably vary from
vendor to vendor.

I don't remember an IPC document with information like that.
There is a list of defects in the IPC-7912 and IPC-9261. They are needed
for DPMO calculations. Defects must be associated with the component,
the placement, the termination, or the assembly. For example, a solder
bridge is a termination defect while a solder balls and splashes are
assembly defects.
