By any chance -- has anyone ever put together a matrix of IPC Defects vs. Possible Root Causes?
We are considering making one for in-house to help standardize data collection and root cause identification
so that we can better ID when we need to open a Corrective Action.

If anything like this exists please let me know -- it may help save lots of time.
