Steve, your right concerning prototypes.  We struggle with this topic.  Mainly due to prototypes are at many differing stages in documentation.  It is a hard process to control.  Also consider that most proto's do result in future production so you'll have to do the doc's and process set-up at some time any ways.  It is nice to be able to look back and see what you did not try to remember what happened.  

Maybe a happy medium.  Enough doc's so you can recreate most of the process but open enough to make time to customer satisfactory.
