Hi All,
This has been a very interesting thread to follow.

Here's my 2 cents:

1. When possible, speak to operators instead of managers or your
"tourguide".  Operators' input often describes the organization more
accurately.

2.Specific board shop questions:
   A. Ask the drill operator about drill bits.  How often are they replaced
& how are they organized
   B. Ask the layup operator (multilayer process) about prepreg shelflife &
storage.  How do they know if it's too old?
   C. Ask the plating operator (or Analytical Dept)about bath analysis.  How
do they keep all those chemistries in spec?
   D. Ask the silkscreen & soldermask operators about ink shelflife & take a
peek at their materials.
   E. Ask the Sales/ Order Entry people- "Who actually reads/reviews my PO"?
   F. Ask the Sales/ Engineering/ or CAM people- "When I order a new
revision, how do you ensure that I don't receive the
       previous revision"?
   G. Ask everyone to point out the applicable procedure in their QA
Manuals.
   H. Ask everyone what types of problems/ defects they encounter with their
process/ product or equipment.

Answers to most of these questions should be backed up by documentation of
some sort: procedures; expiration dates; analytical test records; PO review
signatures; etc. (that's your objective evidence).

There are a million possible questions- these are just some that I've heard
various (& often surprising) answers to.
