Mouse bites on inner layers are caused by etch undercutting the photoresist.
The root cause will either be dust during lamination (visible by holding the
panel at an angle after laminating) or poor resist adhesion due to : poor
surface prep, too little topography on the copper, scratches, or dented
copper foil.
