We have a system that works. The sign off is done by the next operation who
takes ownership of the product. In our rather simple line test validates
pick and place set-up. If production places 10K wrong value resistors test
will be re-working 10K assemblies. I can assure you that the test department
carefully scrutinizes each set-up. Since we try for a pull system test never
drags their feet, first article assemblies are priority one.

Product documentation (just another process) requires signatures from
Design, Production, Procurement, etc. Each function has a vested interested
in seeing the documentation correct.

In my opinion the key to effective sign off is to have the person doing the
checking be motivated, that is they will personally suffer if things are not
correct. Avoiding bad things is a part of human nature, and even though it
is negative reinforcement the individual has no one to blame but himself
when the punishment comes down. This not to say that an independent third
party can't be effective, I just think it isn't required with the proper
motivation.

That check off/reviews/validation are required should be self evident.
Humans are fallible, biased, and myopic about our own opinions. We all need
a sanity check, even though I try to get each document signed off with no
red lines I worry when no one finds any errors. Did they actually read the
thing?
