Carl,
My take is the following

The note " As an exception to fill requirements of Table 6-2 on PTH's ......."
Is explaining why you can have a 50% fill if the following requirements are met. They have added a note about 100% solder fill because many product require thermal shock testing.  And that the user is responsible for identifying these situations.

Question 

 1. If the standard can not be determined how could you accept the call.
 2. Why would you quote the standard if you are not able to use the standard.

I believe what you really what to know is

 If you are not able to judge if there is a 50% fill but all other standards are met is the part acceptable.

Answer
  That is why they have added the note: 
   Less than 100% solder fill may not be acceptable in some application ............................"
They have placed the decision in your hands.

Looking for the groups opinion(s) on the vertical fill requirement stated on
page 6-7 of IPC-A-610C.

Is the exception pertaining to the Defect Class 3?

Therefore making the 50% fill acceptable for class 3, PROVIDING the 360
degree and 100% wetting of barrel walls lead on the secondary side, and
surrounding PTH's meet the requirements of table 6-2.

What is the opinion of all in the above case "IF" you can not actually see
into the PTH to the depth of the 50%, but the soldering is acceptable and
meets all the requirements otherwise?
