Unfortunately there is no one-size-fits-all answer for cost on b&b vias. The
cost multiplier is dependant on the number of extra processing steps
required, the materials used, and the type of drilling used to make the
vias.  If you have blind vias for pad cap on a double sided smt assembly,
with blind vias both sides going in only 1 layer that can be mechanically
drilled, and buried vias that run from layer 2 to 1 layer in from the
secondary side, the cost adder is somewhere between 10 and 25%. As your
design becomes more complex the cost adder goes up and it can become a true
cost multiplier (think spiral vias [hideous scream in background followed by
much evil chuckling >;->]).
