Well, 

I had posted a question earlier this week and Steve was kind enough to post 3 photo's on his website titled delam 1, 2, and 3. 

I have had the PCB in question cross sectioned and found out what it isn't.  It isn't delam, blistering, pink ring, or haloing.  We don't have a definitive answer except the PCB meets all of the requirements and there has been no notable reduction in reliability.  

Thanks to all who responded and all who gave me some really good places to look and learn more about this topic.
