I dealt with this issue about two years ago.  Some of the voids we had found were quite large and exposed the internal circuitry but others were very small and didn't expose the internals.  We ended up making a size maximum which at that application was .020".  If the part had a void that was larger than that it was unacceptable but if it was smaller than that we allowed a small patch.  The application wasn't affected by patches.  We were more worried about entrappment through wave and wash.
