Hi Ken,
     It's been a while since I quoted layout, but here are some basic
considerations I recall:
1. # parts
2. # pins
3. Board square inches (for figuring density)
4. # nets
5. design considerations such as placement, high speed, daisy chain, matched
pairs, controlled impedance, etc. (there are others)
6. format of engineers' inputs (compatible net list, schematic and parts
list files) or Ugh- hard copy.
7. component data sheets provided?
8. delivery time
