If I understand your question the fabricator delivered
30 'arrays' or 'pallets'  and X'd out (rejected) 1
piece per array (pallet).

Sometimes it happens.

Assuming this is an artwork defect (a trace is open or
shorted) the defect could likely be caused in the
photo imaging area where a film is scratched or debris
is shot onto the panel.

For a short run, this type of fallout is not atypical
and depending on the reason for the rejected boards
does not necessarily reflect poorly on the acceptable
boards.
