Mark,
Recently, scoured IPC-D-235A and pull examples of their notes and some
requirements out, as well as some answers I got from this forum.
In the files area of our forum homepage I place a "Modular notes system"
anyone may feel free to use these.
I worked hard on these to make them as complete as possible and cover
most areas.
These point that you ask about are covered in there.
