Brian,

Yes as you mentioned this requirement is not common (relative term) and it it
rather tricky to accomplish without ripping off the plating from the holes.
We at Accurate Engineering can and are doing brds with those requirements
where no lifted plating is allowed.  These are MULTI-LAYER BRDS WITH
CONNECTIONS TO THE EDGE OF THE BRD VIA HALF-HOLES FROM INNER-LAYERS.
