Stephen,

Another resource would be the ISO9001 standard. From a
practical point as a Customer QE and Product/Process
QE I handle these items as follows.

Root-Cause - Why something happened. Typically you
can get to root-cause by asking "Why? " five times.

e.g.1.  Why did the board short?
           Because it had too much solder paste.
           2.  Why did it have too much solder paste?
                The old .008" thick stencil was used?
                 3.  Why was the old stencil used?
                       It had not been recycled?
                      4. Why was it not recycled?
                          The recycling center failed
to pick up.
                           5. Why did they not pick
up?
                               Recycling manager
forgot to call
                                for the pickup.

Root-Cause was human error, failure to call the
recycling vendor for the pickup,

Corrective Action - what is done to correct an issue.

In the above example, the recycling center offered to
do a pickup at specified intervals.

Manufacturing was reinstructed to verify the tooling
revision prior to using it.

In-House recycling created a bright green label for
tooling to be recycled.


Off Specs - depending on the direction the material is
flowing in out of spec items and waiver are handled
basically the same way.

If material is coming to you from a supplier, they
should request that you agree to "waive" what ever
"off spec" detail they are concerned about.

If you are going to use off spec items in your process
you may need to "waive" something in your process.

If the matieral is going out from you to a customer,
you  may need to obtain a waiver or what some people
call a concession from their customer to allow for
shipment of substandard or off spec material.

The waiver is typically a form or form letter which
details discrepancy to be accepted and the quantity to
be accepted.

Example: A lot of plastics has been made whose color
does not completely match the spec. The supplier will
ask their customer to accept the out of spec coloring.
The customer will adjust their process to alert
inspection point to allow for the use of the material.
