Jason,

Coming from years of our UIC Axial machine I would vote for all of the
above!

Actually it could happen at any of the process steps, however the most
likely would be at the lead bending in the formers or at the lead cut
process. That is where the 'pull' is put on the lead.

However if the depth stop or the z-span is not correct the tips could
actually come in contact with the glass and fracture it.
