I don't think you are missing anything and you should change your criteria.
Solder icicles and spikes which do not violate minimum electrical spacing or
mechanical height are not rejectable even for high reliability Military
standards like MIL-STD-2000A.  They need not be reworked but they should be used as process indicators.  The process parameters should be adjusted to correct a
chronic icicling or spiking problem.
