Try www.aiag.org and do a search for PPAP.

They got manuals the big 3 approve of for developing and maintaining QS9000
requirements.

What I've done in the past is generate the same type forms shown in the
book(s) using MS Excel, set up a template and things work pretty smooth
after that.
