What are people doing?  Well, it depends on what you've got set up and what your customer demands.  We don't mark on the boards, but record results on "traveller" sheets that go with the CCA through the process.  Customer required, and the sheets get stored for some number of years (I'm not sure how long).

In a previous life at another employer we did indeed use a barcode and scanner system.  A most excellent system, if you have the volume to justify it.  Everything was barcoded, and at every process step it had to be scanned.  Not only did it record results (pass/fail), operator, and time, the system was set to check for the downstream operations.  Thus one could not (for example) do final inspection on a board that had not passed test.  And the best part - it wouldn't print a shipping label unless everything was passed OK.

In a former position we set up a sequence of three stamps, and maintained a
log book on the shop floor by serial number. Boards were stamped, post
solder inspection, test, and final. We had a system that relied on 100%
inspection.
In a system of lot samples. I think I would continue the practice of
stamping those members of lot that were choosen for inspection. and maintain
the log by lot/serial.

I am sure that a good system could be developed using bar-codes and
scanners. In an envirionment with MRPII or an ERP system this would make
more sense.
